Requirements For Category C Documents

CATEGORY C Documentation:

An applicant can provide evidence of formal accommodations or support services provided in a post-secondary academic setting within the past 18 months.

• This evidence should be printed on the institution’s letterhead.
• The documentation should specify the specific accommodations or support services that were approved and the basis for the approval, including what documentation the institution relied upon, and copies of that documentation if still available.
• If the applicant received different accommodations at different times, the documentation should explain why changes were made.
